1. Agreement to Terms
These Terms of Use (“Terms”) are a legal agreement between you (“you” or “User”) and the individual developer of ChronicOS. By downloading, installing, accessing, subscribing to, or using the ChronicOS mobile application (“App”), you agree to these Terms and to our Privacy Policy, which is incorporated by reference.
If you do not agree, do not use the App. If you use the App on behalf of another person (for example, as a family caregiver), you represent that you have authority to bind that person to these Terms and that you may lawfully manage or share their health information as you configure in the App.
Version 1.0.0 scope. At launch, the App provides manual health journaling, voice symptom entry (including an offline demo path), on-device rule-based visit preparation, medication and lab tracking you enter yourself, rule-based alerts, visit prep exports, optional cloud sign-in, and optional family and caregiver features. Optional cloud AI through OpenAI (text generation and embeddings) consumes Health AI Points only when that cloud processing runs. Patient Portal (Coming Soon) (automatic import from U.S. health systems such as Epic MyChart) is displayed in the App and is not active in version 1.0.0.
Where required by law, we request your consent before optional processing (for example, anonymous analytics, personalization, cloud sync, live voice transcription, or AI features), as described in the Privacy Policy and in-app consent screens.
2. License Grant
Subject to your compliance with these Terms and applicable App Store rules, we grant you a limited, non-exclusive, non-transferable, non-sublicensable, revocable license to install and use one copy of the App on Apple-branded devices you own or control, solely for your personal, non-commercial health organization and education purposes.
You may not:
- Copy, modify, adapt, translate, or create derivative works of the App
- Reverse engineer, decompile, or disassemble the App except where prohibited by law
- Rent, lease, lend, sell, redistribute, or sublicense the App
- Use the App to provide medical care, triage emergencies, or replace professional clinical judgment
- Scrape, mine, or use automated means to extract data from the App or its services
- Remove proprietary notices or circumvent security, encryption, usage limits, or subscription controls
- Access another user’s account or health data without authorization
- Attempt to enable, bypass, or reverse-engineer disabled features (including Patient Portal Sync before it is officially released)
- Use the App in violation of law or third-party rights
3. Not Medical Care; HIPAA; Emergency Disclaimer
ChronicOS is a personal health organization and educational tool for adults managing chronic conditions. It is not a medical device, healthcare provider, telemedicine service, insurer, or substitute for professional medical advice, diagnosis, or treatment.
Rule-based alerts, medication interaction scans (local and optional remote), lab trend charts with clinical reference bands, visit-preparation summaries, between-visit coaching, alert-center signals, and AI-generated content may be incomplete, outdated, or incorrect. You alone are responsible for decisions you make with your licensed clinicians.
HIPAA notice: Unless we expressly enter into a Business Associate Agreement with you or your organization, the Developer is not a HIPAA-covered entity or your healthcare provider. The App is offered as a consumer personal health record tool. You choose what to enter and with whom to share exports or caregiver access.
In a medical emergency, call 911 (U.S.) or your local emergency number immediately. Do not rely on the App or email support for urgent or life-threatening situations.
4. App Features; Free and Paid Services
4.1 Always free (no subscription required)
Core features available without a paid subscription include, among others:
- Multi-condition home dashboard, condition trend charts, and alert center
- Symptom and flare tracking (text and optional voice entry, including offline demo sample)
- Unified health timeline
- Medication list, local interaction / safety scanning, reminders, and side-effect diary
- Doctor and visit records; on-device visit preparation (rule-based, free)
- Lab trend narratives, medication interaction explanations, and correlation insights (on-device, free)
- Between-visit Health Coach (weekly goals by disease combination)
- Rule-based health and medication safety alerts
- Demo mode (View Demo on the welcome screen) with local sample data
- Basic monthly Health AI Points allowance on the free tier (approximately 20 points, as displayed in-app) for optional cloud OpenAI features
4.2 Premium and Family subscriptions
Optional auto-renewable subscriptions unlock expanded entitlements and higher monthly Health AI Points allowances for optional cloud AI through OpenAI — for example specialist/provider summaries, full trend orchestration, cloud voice symptom structuring, and OpenAI embedding-based memory indexing. Subscriptions do not charge Health AI Points for on-device rule engines such as visit prep, lab narratives, medication interaction explanations, correlation analysis, or Health Coach.

Premium includes unlimited active conditions and doctors (subject to fair use), visit prep PDF export, and approximately 200 Health AI Points per month. Family includes everything in Premium, household profiles (up to 6 patients on one device), family dashboard and patient switcher, remote caregiver invitations with scoped permissions, and approximately 500 Health AI Points per month. Exact entitlements, limits, and point costs per AI action are shown in the App and may be updated with notice where required by law or store policies.
4.3 AI Boost Packs (consumables)
One-time consumable in-app purchases add Health AI Points without changing your subscription tier:

Health AI Points are virtual credits consumed only when optional cloud AI processing through our backend and OpenAI runs successfully. They have no cash value, are non-transferable, and do not roll over except as stated in-app. On-device rule engines, local fallbacks, and core tracking features do not consume points.
4.4 Free tier limits
The free plan may limit the number of active conditions (1), care-team doctors (1), and household patient profiles (1), as displayed in-app. Premium and Family plans remove or expand those limits as described above.
4.5 Optional cloud sync and remote caregivers
Sign-in and cloud sync are optional. When enabled, encrypted health payloads may be stored on our backend (see Privacy Policy). Remote caregiver access requires authenticated accounts on both sides and shares data only according to permission tiers you assign. You are responsible for invitations you send and for revoking access when no longer needed.
4.6 Patient Portal (Coming Soon)
The App includes a Patient Portal (Coming Soon) screen describing planned integrations with major U.S. electronic health record systems (for example, Epic MyChart, Cerner / Oracle Health, and athenahealth). In version 1.0.0:
- No OAuth, FHIR, or patient portal connection is activated. The App cannot log in to your patient portal or download portal records.
- No portal credentials are collected, stored, or transmitted by us in this version.
- Waitlist (optional): You may join a waitlist by entering an email address stored locally on your device, or by emailing us at hello@familymedvault.com as described in the Privacy Policy.
- Future releases: When Patient Portal Sync becomes available, we will update these Terms and the Privacy Policy, request any additional consent required by law, and describe applicable third-party health system terms before enabling live connections.
No reliance on unreleased features. Descriptions of Patient Portal Sync are forward-looking and for informational purposes only. You agree not to rely on Beta / Coming Soon features for clinical decisions or data completeness in version 1.0.0.
5. Artificial Intelligence and Voice Features
The App includes on-device rule engines and optional cloud AI through OpenAI (text generation and embeddings via our backend). AI outputs are informational only and may contain errors. You must independently verify AI content before relying on it for any health decision. We do not integrate Google Gemini or other third-party generative AI providers in version 1.0.0.
5.1 Free on-device (no Health AI Points)
- Rule-based visit preparation, lab expanded explanations, medication interaction “why it matters” narratives, correlation analysis, Health Coach, medication safety scanning, alert-center rule engine, and demo mode
- Offline demo path: Try demo sample in Voice Symptom uses sample text without uploading audio and does not require sign-in or server configuration
5.2 Cloud OpenAI (Health AI Points)
- Health AI Points are consumed only when optional cloud OpenAI processing completes successfully, as shown in-app (typical costs: specialist summary ~5, trend orchestration ~5, voice structuring ~1 when LLM extraction is used, memory embedding index ~5)
- Live voice transcription and structuring: Requires microphone permission, sign-in, and a network connection when you initiate recording; points apply only when cloud LLM structuring runs
- We may modify, limit, or discontinue AI models or features at any time
By using cloud AI or voice features, you grant us a limited license to process the content you submit solely to provide those features through OpenAI and our backend.
6. Subscriptions, Billing, and Refunds
Purchases are processed by the Apple App Store. We do not collect or store your full payment card number. Payment will be charged to your Apple ID at confirmation of purchase.
- Auto-renewal: Subscriptions automatically renew unless canceled at least 24 hours before the end of the current billing period. Your account will be charged for renewal within 24 hours prior to the end of the current period at the price shown in the App Store.
- Manage / cancel: Settings → Apple ID → Subscriptions → ChronicOS
- Restore purchases: Available from Profile → Health AI Points / Subscription in the App where shown
- Deleting the App does not cancel an active subscription
- Price changes: We may change subscription prices with notice as required by Apple or applicable law; continued use after the change may constitute acceptance where permitted
- Refunds: Refund requests are handled by Apple under its policies. Contact us at hello@familymedvault.com for account or entitlement help; we cannot override App Store refund decisions
- Trials and promotions: Any free trial or promotional offer converts to a paid subscription unless canceled before the trial ends, as disclosed at purchase
- Taxes: Prices may exclude applicable taxes, which are collected by Apple where required
By subscribing or purchasing consumables, you acknowledge that you have read and agree to these Terms and the Privacy Policy.
7. Account, Cloud Sync, Family Profiles, and Caregivers
You may use much of the App without an account. Optional sign-in enables cloud backup or sync and remote caregiver collaboration. You are responsible for safeguarding credentials and for all activity under your account.
Family profiles: On a Family plan, you may create multiple household member profiles on your device. Each patient’s data is stored in a separate local namespace unless you enable cloud sync. Without an active Family subscription, household features beyond a single patient profile are not available.
Caregivers: If you invite caregivers, you choose permission levels (full access, medication only, or appointment access). You represent that you have the right to share health information you disclose to caregivers. We are not responsible for misuse by persons you invite or for their independent use of exported data.
Patient Portal Sync: As described in Section 4.6, portal import is not available in version 1.0.0. When enabled in a future release, connections will import data you authorize from third-party health systems, and their terms and privacy policies will apply to those connections.
We do not guarantee recovery of data after device loss, factory reset, or uninstall if you did not enable cloud sync or maintain your own backups.
8. Your Data and Responsibilities
You agree to:
- Provide information that is accurate to the best of your knowledge
- Use the App only for lawful purposes and in compliance with applicable health-information laws
- Not upload content that infringes others’ rights, violates privacy, or contains malware
- Not attempt to access other users’ data without authorization
- Not provide patient portal passwords or credentials to us by email or support channels
- Not use the App if you are prohibited under applicable export-control or sanctions laws
You retain ownership of health content you enter. You grant us a limited, worldwide license to host, encrypt, process, transmit, and display that content solely to operate the App, provide optional cloud sync and caregiver features, and fulfill AI or voice requests you initiate.
9. Privacy
Our collection, use, disclosure, and retention of personal information is described in the Privacy Policy. You may export local data as JSON and delete local data from Profile → Settings & privacy. To request cloud deletion, email hello@familymedvault.com with subject ChronicOS Privacy Request.
For version 1.0.0, we do not collect health data from third-party patient portals because Patient Portal Sync is not operational at launch.
10. Intellectual Property
The App, including its software, design, text, graphics, logos, and documentation (excluding your user content), is owned by the Developer and protected by copyright, trademark, and other intellectual property laws. These Terms do not grant you any rights to our trademarks or brand features.
Feedback or suggestions you provide may be used by us without restriction or compensation to you.
11. Availability and Changes
We may modify, suspend, or discontinue any part of the App at any time, including features, AI models, subscription benefits, point allowances, or planned integrations such as Patient Portal Sync. We do not guarantee uninterrupted or error-free operation. Maintenance, updates, and third-party outages (including cloud, AI, or speech providers) may affect availability.
We may update these Terms. Material changes will be posted at this URL with a revised “Last updated” date. For material changes, we may provide additional notice in the App or by email where appropriate. Continued use after the effective date constitutes acceptance, except where law requires additional consent. When Patient Portal Sync or other material features become live, we will update these Terms before or at activation where required.
12. Age and Children
The App is intended for adults managing their own or family members’ health information and is not directed to children under 13 (or under 16 in certain jurisdictions). You must be at least the age of digital consent in your jurisdiction to accept these Terms. If you are under 18, you may use the App only with involvement of a parent or legal guardian who agrees to these Terms on your behalf.
13. Disclaimer of Warranties
TO THE MAXIMUM EXTENT PERMITTED BY APPLICABLE LAW, THE APP AND ALL CONTENT, AI OUTPUTS, ALERTS, COACHING, CHART REFERENCE BANDS, BETA / COMING SOON FEATURE DESCRIPTIONS, AND RELATED SERVICES ARE PROVIDED “AS IS” AND “AS AVAILABLE” WITHOUT WARRANTIES OF ANY KIND, WHETHER EXPRESS, IMPLIED, OR STATUTORY, INCLUDING IMPLIED WARRANTIES OF MERCHANTABILITY, FITNESS FOR A PARTICULAR PURPOSE, TITLE, ACCURACY, AND NON-INFRINGEMENT.
WE DO NOT WARRANT THAT THE APP WILL BE UNINTERRUPTED, SECURE, ERROR-FREE, OR THAT AI OUTPUTS, ALERTS, INTERACTION SCANS, COACHING, OR FUTURE PATIENT PORTAL SYNC WILL BE ACCURATE, COMPLETE, TIMELY, OR SUITABLE FOR ANY CLINICAL PURPOSE.
Some jurisdictions do not allow exclusion of implied warranties; in those jurisdictions, the above exclusions apply to the fullest extent permitted.
14. Limitation of Liability
TO THE MAXIMUM EXTENT PERMITTED BY LAW, IN NO EVENT SHALL THE DEVELOPER (AN INDIVIDUAL OPERATOR) BE LIABLE FOR ANY INDIRECT, INCIDENTAL, SPECIAL, CONSEQUENTIAL, EXEMPLARY, OR PUNITIVE DAMAGES, OR ANY LOSS OF PROFITS, DATA, GOODWILL, OR HEALTH OUTCOMES, ARISING OUT OF OR RELATED TO YOUR USE OF OR INABILITY TO USE THE APP, YOUR RELIANCE ON BETA / COMING SOON FEATURES, OR YOUR USE OF AI OR VOICE OUTPUTS, EVEN IF ADVISED OF THE POSSIBILITY OF SUCH DAMAGES.
TO THE MAXIMUM EXTENT PERMITTED BY LAW, THE DEVELOPER’S TOTAL AGGREGATE LIABILITY FOR ALL CLAIMS ARISING OUT OF OR RELATING TO THE APP OR THESE TERMS SHALL NOT EXCEED THE GREATER OF (A) THE AMOUNTS YOU PAID TO US (OR THROUGH THE APP STORE FOR THE APP OR IN-APP PURCHASES) IN THE TWELVE (12) MONTHS BEFORE THE EVENT GIVING RISE TO THE CLAIM, OR (B) FIFTY U.S. DOLLARS (USD $50.00).
Nothing in these Terms limits liability that cannot be limited under mandatory consumer protection, privacy, or health-safety laws in your jurisdiction (including certain GDPR/EEA rights).
15. Indemnification
You agree to defend, indemnify, and hold harmless the Developer from any claims, damages, losses, liabilities, and expenses (including reasonable attorneys’ fees) arising from your misuse of the App, your violation of these Terms, your health content, your sharing of data with caregivers or third parties, your unauthorized sharing of another person’s health information, or your violation of applicable law, except to the extent caused by our gross negligence or willful misconduct where such limitation is prohibited by law.
16. Termination
You may stop using the App at any time and delete local data from Settings & privacy. Cancel active subscriptions through your Apple ID subscription settings; deleting the App alone does not cancel billing. We may suspend or terminate access if you breach these Terms, if we discontinue the App, or if required by law or App Store policies.
Sections that by nature should survive (including intellectual property, disclaimers, limitations of liability, indemnity, governing law, and Apple provisions) will survive termination.
17. Governing Law and Disputes
These Terms are governed by the laws of the State of California, United States, without regard to conflict-of-law principles, except that mandatory consumer protection laws in your country of residence may apply to you.
Any dispute shall be resolved in the state or federal courts located in California, and you consent to personal jurisdiction there, unless applicable law requires otherwise. You may also have the right to bring claims in your local courts under mandatory consumer laws.
18. Apple App Store Terms (Required for iOS Users)
If you downloaded the App from the Apple App Store, you acknowledge and agree:
- These Terms are between you and the Developer only, not Apple Inc. (“Apple”).
- The license granted to you is limited to a non-transferable license to use the App on Apple-branded products that you own or control, as permitted by the Apple Media Services Terms and Usage Rules.
- Apple has no obligation to furnish maintenance or support for the App. Technical support contact information is set forth in Section 20 and at support.html.
- In the event of any failure of the App to conform to any applicable warranty, you may notify Apple for a refund of the purchase price (if any) paid for the App; to the maximum extent permitted by law, Apple has no other warranty obligation with respect to the App.
- Apple is not responsible for addressing any claims by you or any third party relating to the App or your possession or use of the App, including (i) product liability claims, (ii) any claim that the App fails to conform to any applicable legal or regulatory requirement, (iii) claims arising under consumer protection or similar legislation, and (iv) claims with respect to intellectual property infringement.
- Apple and its subsidiaries are third-party beneficiaries of these Terms and may enforce them against you as a third-party beneficiary.
- You represent that (i) you are not located in a country subject to a U.S. Government embargo or designated as “terrorist supporting,” and (ii) you are not listed on any U.S. Government list of prohibited or restricted parties.
- You must comply with applicable third-party terms when using the App (for example, your wireless data service agreement).
- Subscription and consumable purchases are subject to Apple’s payment terms. Apple is not responsible for the App’s content, AI outputs, health information you enter, or Beta / Coming Soon feature descriptions.
19. General
- Entire agreement: These Terms and the Privacy Policy constitute the entire agreement regarding the App.
- Severability: If any provision is unenforceable, the remainder stays in effect.
- No waiver: Failure to enforce a provision is not a waiver.
- Assignment: You may not assign these Terms without our consent; we may assign them in connection with a sale of the App or assets with notice where required.
- Individual developer: The App is offered by an individual developer, not a hospital, clinic, insurer, or clinical entity. Our resources and support capacity are limited compared to enterprise healthcare organizations.
- Force majeure: We are not liable for delays or failures caused by events beyond our reasonable control.
- Language: These Terms are written in English. Translated in-app text is for convenience; the English version controls to the extent permitted by law.
